HCS School Board Recognition Form

The School Board loves to celebrate the success of our students and staff!

We honor these achievements at School Board meetings throughout the school year. To help us do this, we collect information about special accomplishments by people in our schools.

These achievements can include (but aren't limited to):

Winning at the Science or Social Studies Fair (regional or state)
Awards in sports at the state, regional, or conference level
Winning art contests
Music, choir, or band honors (like All-County or All-State)
Local or national awards
Big scholarships or special college acceptances
National teaching awards or certifications
Earning a new degree, certification, or National Board Certification

Please note: This form is intended for official recognitions based on awards or accomplishments. If an employee would like to give a "shout-out" to a teacher or staff member for their great work, please use the Employee Shout-Out Form in the monthly staff newsletter.
